Hartshill Medical Centre is iparticipating in the Summary Care Records Service which makes patients' medical records immediately accessible from anywhere in the country. It will allow health care professionals access to key information when treating patients in locations such as Accident & Emergency, GP Out of Hours Service and Walk-In Centres. To start with the SCR will only contain details of allergies, recent prescriptions and previous bad reactions to medicines. In the future more information may be addedd to your SCR such as significant illnesses and conditions, important test results and immunisations.
Access to your SCR is only available to NHS clinical staff involved in treating you. There are strict measures in place to protect your information. To ensure security, only staff with a smartcard, PIN and legitimate reason for accessing the information can access the system. If a healthcare professional needs to view your SCR they will first ask your permission.
You have a choice - you can choose not to have a SCR. If you are happy to have a SCR, you do not need to do anything as the SCR will be created automatically. If you do not wish to have a SCR, you will need to make us aware of this by completing an opt out form, available from the practice or see link below. You can change your mind at any time and choose to rejoin at any time. Please think carefully, the implications may be significant. The initial upload of records took place in mid June 2009.
