Care Of Our Frail Patients

As part of our general care for all our patients, we also carry out consultations with patients who have been identified as being at a higher risk of an emergency hospital admission.  There are a number of factors that can be associated with this:

  • Age (the risk increase over 65 years)
  • Poor mobility
  • Cognitive impairment (Confusion, memory loss, agitation)
  • History of falls
  • Medical Conditions
  • Reduction in vision and hearing
  • Poor nutritional status due to physical factors (lack of transport to shops, inability to use equipment for preparing/cooking etc)
  • Emotional distress or depression
  • Social isolation

 

These appointments are usually carried out by our Health Care Assistants who will assess the patient’s individual needs and help to access safe and effective services if required.
