Should you move house or leave the country/area either temporarily or permanently it is vital that you inform the practice of your new address. It is imperative that we keep our records as up to date as possible. if your new address is outside our practice area you will be required to register with a new practice.

Likewise, if your home or mobile phone number is changed please inform us  as soon as possible.
